For additional information or questions,

call us at (909) 382-7185. Request drop-offs and/or pick-ups are made by appointment only. Please call ahead to schedule a time for a Release of Information Representative to meet with you.

The department is open Monday - Thursday from 9:00am - 4:00pm. It is closed for lunch from 12:00pm - 1:00pm.
